Outcomes Matter Innovations, LLC (“OMI”, “we”, “us” or “our”) takes very seriously its obligation to protect the confidentiality of your personal information. OMI develops and provides a web-based portal for patient care management, known as OMI PULSE for use by healthcare providers, health insurance carriers, and other healthcare-related staff end users.

Introduction

This Privacy Policy and Notice describes how OMI’s OMI PULSE web-based portal for providers, insurers and other healthcare-related end users use, store, and transmit information and data. OMI may modify this Privacy Policy and Notice at any time and shall post the new version here, with the date of the most recent revision posted at the top. Your continued use of OMI PULSE after the posting of any new or revised Privacy Policy and Notice shall constitute your consent to such terms, as modified.

Your Personal Information

Connections to Healthcare Providers and Health Insurance Carriers

To use OMI PULSE, you must be an employee or other authorized user engaged with a healthcare provider or health insurance carrier who has contracted with OMI for use of OMI PULSE. Your use of OMI PULSE may also be subject to your employer’s policies and terms. You understand that while connected or attempting to connect to OMI PULSE, we may collect, store, process, maintain, upload, sync, transmit, share, disclose, and use certain data and related information, including but not limited to information or data regarding the characteristics or usage of your device, system and application software, and peripherals as well as your personal information, location data, and other content as more fully described herein.

Information We Collect

We may collect information about you including non-personally identifiable information and/or “Personal Information,” which is information that may identify, relate to, describe, or be capable of being associated with or reasonably linked, directly or indirectly, with a particular identified or identifiable person or household.

Without limitation, the following types of information are not considered Personal Information for purposes of this Privacy Policy and Notice: PHI covered by HIPAA, information derived from PHI that is de-identified in accordance with HIPAA, and PHI we handle in our capacity as a Business Associate to a Covered Entity.

Using Third Party Tools and Features

If you use any third-party tools and features, such as third-party speech-to-text dictation or third-party video, your use of those features is subject to the terms and policies of those third parties. If you have any questions about those terms or policies, you should contact the provider of the third-party tool.

Currently, we do not link to any third-party websites or services. If, in the future, we add links to such content, your use of any third-party websites or services shall be subject to the terms of such third-party’s terms of service and privacy policy.

How We Protect Your Personal Information

The security of your Personal Information while using OMI PULSE is very important to us. We employ a variety of technical safeguards to protect the confidentiality, integrity, and availability of your personal information including supporting Transport Layer Security (TLS)/Secure Sockets Layer (SSL) certificate technology and encryption.

We retain your Personal Information only for as long as is necessary for our legitimate business purposes. Additionally, we may continue to store your Personal Information contained in our standard back-ups. This applies to all categories of Personal Information in use by us.

How You Can Help Protect Your Personal Information

If you are using an account for which you registered and chose a password, you should not divulge your password to anyone. We will never ask you for your password in an unsolicited phone call or in an unsolicited email. Also, remember to sign out of the OMI PULSE platform and close your browser window when you have finished your work.

Please note that unencrypted email is not a secure method of transmission, as information in such emails may be accessed and viewed by others while in transit to us. For this reason, we prefer that you not communicate confidential or sensitive information to us via regular unencrypted email.

Children's Privacy

We do not knowingly collect information from children (as defined by COPPA) and we do not target our Platforms to children. If we learn that we have collected any information from children, we will delete it.
